Responsible for demonstrating proficiency in all pharmacist functions, such as interpreting, transcribing, or verifying medication orders. Attend and actively participate in daily multidisciplinary and bedside rounds, complete drug utilization reviews to promote rational drug therapy, lead and develop clinical services, and mentor and educate staff to ensure accurate and appropriate documentation of clinical activities as needed.

 
Responsibilities for Internal Candidates  
 
  • Reviews, interprets, and inputs treatment orders to ensure accuracy of dosage, treatment schedule, route of administration, and safe administration based on laboratory information and evidence-based practices.
  • Provides and supports clinical pharmacy responsibilities by collaborating with other hospital disciplines to support service lines
  • Lead and develop pharmacy clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as defined by Pharmacy Leadership
  • Provide effective medication and practice-related education and counseling to patients, caregivers, and medical staff
  • Collaborate with medical and nursing staff to optimize the pharmacotherapeutic aspects of patient care including infusion and outpatient medication management.
  • Provides information to staff members and/or patients on medication uses, drug interactions, and potential side effects.
  • Oversees the preparation of treatments, preparation of investigational treatments, prepackaging of medications, and drug returns
  • Maintains records on patient prescriptions, charges, and inventory.
  • Ensure pharmacy technician duties are performed completely, safely and without risk of harm to patients.
  • Analyzes, compares, and evaluates various courses of action; makes independent decisions.
  • Aid in the precepting of pharmacy students and residents
  • Performs all other duties as assigned
 
Qualifications for Internal Candidates  
 
  • Knowledge of and compliance with state and federal pharmacy laws and regulations.
  • Excellent communication skills; in order to provide service to patients, caregivers, and peers in a reliable, responsive, safe, and courteous manner.
  • Provides service to patients, caregivers, and each other in a reliable, responsive, safe, and courteous manner; promote inclusiveness and collegiality by demonstrating respect and professionalism to others at all times.
  • Knowledge of FDA health regulations, pharmacology and legal compliance requirements in order to provide accurate and timely medication education and counseling to patients and other healthcare professionals.
  • Organization and problem-solving skills with the ability to work in situations involving uncertainty and rapid change; deal constructively with mistakes and/or setbacks; demonstrate flexibility.
  • Ability to align all patient care activities and project work with departmental and institutional policies, business model/needs, goals, and vision.
  • Ability to collaborate with physicians, nurses, and all members of the health care team.
  • Thorough understanding of dosage administration and measurement, chemical compounds, medical brands, etc.

Education: 

  • Bachelor’s degree in pharmacy or Doctor of Pharmacy from an ACPE-accredited college of Pharmacy is required

Certifications:      

  • Current TX state license to practice pharmacy, or licensure in TX within 90 days of hire is required Upon Hire
  • Basic Life Support Upon Hire
  • Advanced Cardiac Life Support within 90 Days
